In my 23 years in the business, I have only once had a Surveyor ask for attendance sheets. When I started at my present facility, the Director had the attendance records in the charts. That is not good, she was feeding the Surveyors more information than they needed. We keep attendance books in the Activity Department and I have a sheet where we chart for the whole year on each resident. We do not do individual charting for one on one visits either. This is noted on the back of the attendance sheet and included in the quarterly note. When I did consulting work at another facility, they too had the attendance in the medical charts. The first thing I did was pull them out. It's better to only give the Surveyors the required amount of information and have the rest on hand if they need it. Kathi
kathi, what? you do not keep 1:1 documentation!? You are setting yourself up for a deficiency my friend. When those surveyors come in and ask what you are doing for your bedfast residents, you better have some documentation with specefic one to one interventions to back you up. Standard 1:1 visit documentation should include, the type of activity offered, the lenghth of the visit, and a brief summary of what took place. I know this sucks, but it is what the surveyors want.
