Progress notes

Hello all

I have worked in several facilities in three states through out my career. I have never run into this before...the facility I'm at now..progress notes are only written on the back of the care plan. The care plans are kept in a binder not even at the nurses station, but in a locked office!! There are no progress notes on the charts!!No admission note, no discharge note, no quarterly note...just the note on the care plan. (I am referring to activity progress notes)
Correct me if I'm wrong on this, but aren't progress notes supposed to "flow" and be part of the chart?? Please help
