: : I was told pretty much the same thing. Now, the only residents who get "little or no" are the vegetative or comotose residents. Everyone else, I make an effort to involve them in something, whether independent or group, to get them to the "some" catagory. "little or no" is an indication that you should take action on the care plan. AS long as you've got yourself a good care plan to cover it, I think you should be fine.

: You should mark 'em as you see 'em. The triggers only work if they trigger sometimes. On the other hand, you should make sure they really aren't doing anything like watching TV with a friend or reading when you're not there. Be sure to provide some thingsthings they can do with little supervision should you find a lot of peopel trigger for this.
