"Little or No Activity" on MDS

The DON pulled me aside the other day and said that we have to get together and talk about the residents that are marked "Little or No Activity" because, she said, "That is unacceptable." At one nursing home I worked at, we were also told to never mark that. First off, there are people that are uninvolved for whatever reason. If my professional judgement tells me that this is appropriate, then that is what I am going to mark. It is my belief that having all of the residents marked as "spends some time involved...." is more of a red flag for the state. What is you opinion and has anyone ever been cited either way?
