I have taught my staff to write progress notes each quarter that include the exact programs that the resident attends and how often. In my 23 years in the business, I have only once had a Surveyor ask for attendance sheets. When I started at my present facility, the Director had the attendance records in the charts. That is not good, she was feeding the Surveyors more information than they needed. We keep attendance books in the Activity Department and I have a sheet where we chart for the whole year on each resident. We do not do individual charting for one on one visits either. This is noted on the back of the attendance sheet and included in the quarterly note. When I did consulting work at another facility, they too had the attendance in the medical charts. The first thing I did was pull them out. It's better to only give the Surveyors the required amount of information and have the rest on hand if they need it. Kathi
