I would like to gather addresses of hospital-based skilled units- it's part of a "data-gathering" project for my boss. If you are located in, or now of, a hospital-based SNU/SNF, please send me the address. You may post it here or send it to my email- costerj@rcbhsc.wvu.edu Even if you don't know the address, just send the name of the facility and the city.
