I do a complete assesment for my long term care and short term care residents. This helps me to understand their strengths and needs before I do my Care plan. All the other disciplines get together after we have completed our initial assesments and then we all do an interdisciplinary care plan together. You need to get all the background information you can get before the team meets. I find that most of the time I can help other disciplines with their careplannin just on the information that I have gathered from the resident and families. We are there to know our residents on a more personal level than any of the other disciplines.
