Documentation for Assisted Living

Hello. I was wondering what kind of documentation everyone else does for Assisted Living Communities.
I know that the Regs. in NJ don't have any clear information about documentation for Activities. I just want to cover my butt when the state comes in. Any suggestions would be a big help.
