and the time of day, i have a sensory check off and i also use the comments section for mood and ability to participate,
all of my sensory,1-1 residents have music or books on tape, or other increased sensory going in their rooms while they are awake, also for any resident with wandering, agitation, or sever cog impairment i have interventions that are not only used by my staff but everyone else in the facility, my interventions are on their charts in the rooms as well as a flow sheet to mark when they are used. mY 1-1 flow sheets were made bt me, as i couldnt find a form that actually covered all that we did, hope this helps, you are welcome to e-mail me if you would like to see a sample,
