Your best bet is a generic chart that you can use for each resident.

Example: Leave a space for the Name; Rm #.

In a space provided state the date; type of 1:1 eg sensory; tactile; spiritual; etc.

Include with type of visit: Resident's mood state before, during and after visit. (Also list if there was no reaction to visit.)

State the length of this visit.

Finally, keep all 1:1 visit charts in a single binder for quick and easy reference. This is especially helpful for surveys, care plan meetings, family questions, nursing inquiries, etc.
